If you would like to instruct us, please complete and submit the relevant electronic forms below. Alternatively, print and then complete the relevant documents which can either be faxed or posted to us or, in the case of the first three, sent with your files. If you require any further help please email us or call us on the above number.

Proforma instructions sheet - This will provide us with all of the necessary information relating to your file.

Client information sheet - This will provide us with any background information relevant to your firm's areas of activity.

Terms and conditions - To be read and signed by you prior to our carrying out any work on your behalf.

Courier Request Form - Clients wishing to send us high value cases may qualify* for our free collection and delivery service. Simply print and complete this form and post or fax it to us, or alternatively, send us an email requesting a collection by courier.
* The free service is at the discretion of Deborah Burke Costing Limited and is based on, but not limited to, the value of any files to be sent and the location of the client.
